Entire Physio’s core philosophy is to exceed our customers expectations by providing the best possible holistic health care.
Upon booking an appointment you will receive an email that confirms your appointment. 48hrs prior to your appointment you will also receive a reminder SMS.
Please ensure that if you need to change an appointment, you do so at least 24hrs prior to your appointment.
We understand that illness and personal issues can occur at any stage, therefore if this occurs we are more than happy for you to call our reception and discuss this with them. We are unlikely to charge you our late cancellation fee if this is the case.
If you do not arrive for your appointment, or cancel at the last minute, we will charge you a $55 fee.
This is NOT a waiver, it is part of our duty of care to you that all physiotherapists and other allied health practitioners inform you of any risks associated with professional techniques.
Some therapy techniques such as remedial massage, cupping, dry needling, joint manipulations, traction, or mobilisations have a slight risk of causing injury. Any or all of the above techniques can occasionally cause temporary local swelling, bruising, or transitory increases in the level or distribution of pain and other symptoms. Allergic skin reactions to massage oils, strapping tapes or topical applications are a possibility.
Following the verbal explanation of our therapists examination results and the explanation of therapeutic techniques the therapist feels suits your present condition, you will be asked to give your consent to treatment. You have the right to refuse the treatment the therapist offers at any time. You have the right to a second opinion at any time.
I give permission to the treating therapist to exchange information with the other therapists within this clinic as needed, my doctor and other medical specialists when necessary. I understand that this information will be kept confidential.
